Description of Duties:
This role will primarily involve working with patients with sickle cell disease and other hemoglobinpathies like thalassemia but will also branch out to those with other complex non-malignant blood diseases; will also conduct community events and outreach; coordinate peer and parent support efforts; facilitate greater health education and advocate for patients as they traverse the medical world, especially for transition to adult care for older adolescents and linking patients and families to appropriate psychosocial services.
Applicants must have an Associate's degree in a health related field and the equivalent of two (2) years of full-time experience in community, patient or customer-oriented support or a High School diploma or G.E.D. and the equivalent of four (4) years of full-time experience in community, patient or customer-oriented support.
The preferred candidate will have a Master's degree in Public Health or Social Work.
This is a grant-funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables is required.
